On Pi 5 it's definitely not plug-and-play and took a lot of trial and error to get it working. The key for me was to:
1. Specify IQaudIO DAC Plus in the DAC Model setting of Audio Output under General Playback Options
2. Specify this in /boot/config.txt: dtoverlay=iqaudio-dacplus”
