This is to include copies of passport,bank statement utility bill , phone number. How will this help. They have nothing to compare with. Also, this form states you do not have to use this form. Requests on other media will be accepted. After lots of email message requests I still get replies (from 5 different people) demanding that I must use the form. And there is no alternative.
